The SAC — Smart Access Cavity is a modern approach to endodontic access cavity preparation based on one simple principle:
Remove only what is necessary to see, reach, instrument and disinfect the root canal system — while preserving as much sound tooth structure as possible.
But conservative does not simply mean smaller. A truly conservative access requires a clear understanding of the internal anatomy, careful preoperative analysis, magnification, controlled dentin removal and, above all, the right instruments for the right clinical situation.  In this exclusive masterclass, Dr. Claudio Farnararo will present the principles and clinical workflow behind the SAC.

One of the key elements of the SAC concept is the rational selection of specific burs for each stage of access preparation. During the masterclass, Dr. Farnararo will introduce a dedicated sequence of Microcopy burs, demonstrating how and when they can be used to achieve controlled and predictable access preparation. But the focus will not simply be on which bur to use. For each instrument, Dr. Farnararo will explain the clinical rationale behind the choice:

Why this bur?

Why at this stage?

Where should it be used?

In which direction?

How much dentin should be removed?

What anatomical information are we looking for?

When should we stop?

When should we change instruments?

Understanding the “why” behind the bur selection allows clinicians to understand and adapt the SAC concept rather than simply reproduce a sequence.

FROM RADIOGRAPH TO ACCESS

A predictable access begins before the first bur touches the tooth.  The SAC workflow starts with a systematic analysis of the preoperative radiograph, using the available anatomical information to anticipate canal position, angulation, calcifications, overlapping structures and potential difficulties. Under the operating microscope, this information is progressively translated into a controlled clinical approach. The objective is not simply to create an opening. It is to create the right opening for that specific tooth.

PRESERVE — WITHOUT COMPROMISING DISINFECTION

Minimally invasive endodontics should not mean sacrificing visibility, access or effective instrumentation for the sake of creating a smaller cavity.  The SAC concept is based on a different balance:  Preserve what does not need to be removed.  Remove what is necessary to achieve predictable disinfection. “We shape only what we need to disinfect.”

MEET YOUR EDUCATOR

Dr. Claudio Farnararo

Dr. Claudio Farnararo participated in the annual intercultural program (AFS) and obtained his High School diploma in New Lothrop, Michigan, in 1996. He earned his D.D.S. from the University of Florence in 2004 and subsequently specialized in Oral Surgery at the same institution in 2007.

Dr. Farnararo received specialized training in clinical and surgical endodontics through courses in Italy and abroad led by esteemed speakers. In 2015, he served as a Contract Professor in the specialization course of Endodontics at the Second University of Naples. Additionally, he held the position of Lecturer Professor in the II level Master of Oral Implantology at the School of Medicine and Surgery of the University of Campania "Luigi Vanvitelli" Naples during the academic year 2022/2023.

From 2020 to 2023, Dr. Farnararo was a Member of the Fradeani education team. He also served as the Academic Coordinator of the University Master PGO-UCAM in Naples, focusing on Clinical Microscopic Endodontics and Endodontic Microsurgery for the academic year 2025-2026.

Dr. Farnararo co-authored the chapter "Tools and techniques for whitening non-vital teeth" in the book "Black&White: multidisciplinary protocols of cariology, whitening, and treatment of dental dyschromia," published by Edra and written by Dr. Turrini and Dr. Emiliani.

As a renowned speaker, Dr. Farnararo has presented at national and international congresses in various countries including China, the UAE, Saudi Arabia, Latvia, Romania, Qatar, the UK, Egypt, Syria, and Belarus. He provides consulting services to multinational companies, advising on the development and improvement of equipment and materials dedicated to endodontics.

Dr. Farnararo also organizes practical workshops and hands-on courses both in Italy and internationally. His practice focuses on clinical and surgical micro-endodontics, and he serves as a consultant in selected practices in Naples and Florence.
